PRUSSIAN TRAVEL SOCCER REGISTRATION

The following items are required for Prussians travel soccer registration:
• a wallet size photo (please write name and team, if
known, on back)
• a check payable to KOPSC
On or Before 4/25 Registration Fee is
$145.00
After 4/25 Registration Fee is $155.00
• a copy of a State Birth Certificate (not Hospital
Certificate)
• a completed EPYSA RG-6 Form (see link below)
• a completed KOPSC Medical Release Form (see link
below)
